At Y.A. Transition and Support Homes Inc., we would like to begin by acknowledging that we are on treaty land, referred to as Treaty 6 Territory and that the City of Edmonton and all the people here are beneficiaries of this treaty. Treaty 6 encompasses the lands of the Cree, Dakota, Nakota, Saulteaux (SO-TOE) and the homeland of the Métis Nation. We are dedicated to ensuring that the spirit of Reconciliation and Treaty 6 is honoured and respected.

Our Story

…filling the gap with hope and support

Y.A. Transition and Support Homes Inc. was established in 2024 in response to a critical and unmet need. We witnessed the overwhelming challenges faced by young adults with complex issues aging out of care; the high risks of homelessness, mental health struggles, and isolation. Our foundation was built on a simple, powerful belief: every young person deserves a supported transition into adulthood, equipped with the skills, stability, and confidence to build a future filled with hope.

Our Mission

“is to address the complex needs of at-risk youths. We aim to break the cycle of trauma and family separation, tackle issues related to mental health, drug and alcohol abuse, sexual abuse, homelessness and legal issues. In order to provide a safe place for youth to reach their potential.“

Our Vision

"To empower at-risk youth to confidently transition into adulthood by providing the resources, guidance, and opportunities they need to discover their potential, embrace their purpose, and build a future filled with hope, resilience, and success."

Our Values

  • Holistic Well-being: We believe that for a young adult to be healthy and resilient, the mental, physical, emotional, social and spiritual aspects must be in balance.

  • Trauma-Informed Care: We understand the impact of complex trauma and provide a safe, supportive environment for healing.

  • Person-Centered Approach: We work with youth "where they are at," supporting their personal goals as they transition into adulthood.

  • Cultural Relevance: We are committed to providing culturally relevant supports and honoring the unique heritage of each individual.

  • Unconditional Positive Regard: Every youth deserves to be treated with respect and dignity, characterized by their ability to overcome obstacles.
